Programs Offered Through the Joint Venture

U-M Health physicians have been working alongside Trinity Health physicians and caring for patients in Chelsea for more than four decades. Today, we offer an ever-expanding list of services at Chelsea Hospital, including cardiology and heart failure, family medicineophthalmology, general surgery, orthopedic surgery, and urology.

Inpatient Rehabilitation Program

Trinity Health and U-M Health now offer expanded inpatient rehabilitation at Chelsea Hospital. This expansion of inpatient rehabilitation services will ensure patients recovering from stroke, brain injury, amputation, major trauma, neurological disorders or other complicated medical conditions with rehabilitation needs, receive the best care in the most appropriate setting.

Chelsea Heart Failure Program

U-M Health has also developed a program to provide heart failure patients with all the resources they need for success and to minimize the chance for readmission. The Chelsea Inpatient Heart Failure Service includes optimization of heart failure medications, an engaging education program, close monitoring following discharge, and support from a team including specialists from pharmacy, social work, and nutrition. Patients admitted to the Inpatient Heart Failure Service will be cared for by U-M Health hospitalists on the Michigan Hospitalists Chelsea (MHC) Service.
